Intake Appointments for Behavioral Health Services

To make an appointment with a social worker or psychiatrist at the Gregory P. Shadek Behavioral Care Center at Englewood Health, the first step is a short intake assessment. These assessments are conducted by telephone.

Use our online self-scheduling system below to schedule your intake assessment. After your assessment call, our intake team will refer you to an appropriate social worker or psychiatrist for psychotherapy and/or medical treatment.

There is no charge for these assessment calls, but you will be asked to provide your insurance information.

All intake appointments must be scheduled online. If you have any questions, please call us at 201-292-1400.
